Advance Directives

An advance directive is a legal document that may authorize a designated person to act as your spokesperson if you are unable to make your own medical decisions.

If you already have an advance directive, we ask that you bring that with you when you come in for your preoperative testing or on the day of admission. If you do not already have an advance directive, you may click here to download one in PDF format. The instructions on the form will guide you in completing the advance directive.
